What Is Root Canal Therapy?
Root canal therapy (also called endodontic therapy) treats infection inside the spaces within a tooth’s roots. Those spaces contain pulp: the blood vessels, nerves, and connective tissue that kept the tooth alive during development. Bacteria can invade the pulp through deep decay, a crack, or a chip, triggering inflammation and infection. Once bacteria destroy the pulp’s vascular supply, the body’s immune cells can no longer reach the infection site, allowing bacteria to multiply unchecked.
Left untreated, bacteria and their byproducts can exit through the root tip, damaging surrounding bone and tissue and potentially causing an abscess that won’t resolve on its own. Root canal therapy removes the infected pulp, cleans and shapes the interior of the canals, and seals them with a material called gutta-percha to help prevent reinfection.
Is a Root Canal Painful?
The reputation root canals have for being painful is largely based on outdated information. Modern anesthesia and technique have changed the experience considerably. Local anesthesia eliminates sharp pain during the procedure.
You may notice pressure or vibration, but the sharp pain most people fear is well controlled. It’s also worth noting that much of the discomfort people associate with root canals comes from the infection itself, which the procedure relieves.
For patients who need additional support, anti-anxiety medications can reduce awareness of any remaining sensations, and sedation may be available for those who can’t tolerate treatment while awake.

How Long Does Root Canal Therapy Take?
Most root canals are completed in one or two appointments, each running roughly 90 minutes. The biggest variable is the number of canals in the affected tooth: back teeth (molars and premolars) typically have more canals than front teeth and require more time.
Access to the tooth and how comfortably a patient can hold their mouth open also play a role. Teeth with severe infection or complex anatomy may need an additional visit. The first appointment focuses on removing the infected pulp; any follow-up appointments address final cleaning, canal filling, and sealing.
Does the Tooth Need Further Treatment After Root Canal Therapy?
After root canal therapy, the canals are sealed, but the tooth itself is often weakened by the decay or damage that caused the infection. That weakened area needs to be restored with a filling or other restoration to seal the tooth against bacterial reentry. The larger the area of decay, the more vulnerable the tooth becomes to fracture under normal chewing forces.
A dental crown, which surrounds and reinforces the tooth, is frequently recommended after root canal therapy on back teeth. Skipping or delaying the final restoration increases the risk of the tooth cracking or becoming reinfected, which can undo an otherwise successful procedure. Once the crown is placed, routine hygiene and checkups remain important because the surrounding gum tissue can still develop disease.
