Dental Sealants for Children
What Are Dental Sealants?
A dental sealant (also called a pit and fissure sealant) is a professionally applied plastic coating placed on the chewing surfaces of back teeth to prevent cavities. Those surfaces have tiny pits and fissures that trap food and cavity-causing bacteria. The sealant is a thick liquid that flows into the grooves and hardens, sealing them against penetration by food and bacteria.
When Should My Child Get Dental Sealants?
The teeth most likely to have grooves are molars and premolars. The ideal time to place a sealant is as soon as possible after a tooth erupts. Adult first molars erupt around age 6; second molars and premolars typically appear between ages 10 and 13. The ideal timing varies by tooth, but the appointment itself is straightforward and fits naturally into a routine visit, especially with Dr. Wilson’s patient, unhurried approach with younger patients.
How Long Do Sealants Last?
Longevity depends on several factors. The adhesive bonds best to dry teeth, but isolating a tooth from saliva in a small mouth can be difficult. Back teeth take significant stress during chewing, which can cause a sealant to chip or break out. Even if the visible portion falls away, some material remains lodged in the tooth’s pores and continues to protect it. A damaged sealant should be replaced as soon as possible.
Do Dental Sealants Eliminate the Need for Fluoride?
No. Fluoride and sealants work through different mechanisms on different surfaces. Fluoride strengthens enamel on smooth tooth surfaces and benefits both children and adults. Sealants physically block food and bacteria from the grooved chewing surfaces of molars and premolars. Used together, they provide complementary protection and reduce the likelihood of cavities.
Alternatives to Sealants
Why Do Some Teeth Need Sealants & Others Need Fillings?
Back teeth have grooves on their chewing surfaces that trap food and bacteria, yet are often too narrow for a toothbrush to clean effectively. Bacteria consume the trapped food and produce an acidic byproduct that softens and eats through tooth structure. That softened area, containing food and bacteria, is what we call decay or a cavity.
A sealant applied before bacteria penetrate the grooves can prevent this process entirely. Once bacteria and food have worked their way in, a sealant may not stop further damage. The decay must be removed and the resulting area restored. Small defects can be treated with a flowable resin that reaches into nearby grooves while still resisting biting forces, a procedure called a preventive resin restoration (PRR).
Deeper or wider decay may require a material that can be packed rather than flowed, called a composite resin, in a procedure known as a composite restoration filling.
