How an Immediate Denture Is Made Before Extraction Day
Immediate dentures are fabricated before the extraction appointment, not after. During preparatory visits, while your natural teeth are still present, Dr. Wilson takes impressions, records your bite, and selects tooth shade and shape. It typically takes four to five of these visits to complete fabrication. Because the mold is made from your pre-extraction ridge rather than healed tissue, same-day placement is possible, but fit will shift as healing progresses.
On extraction day, the teeth are removed, and the immediate denture is seated directly. In some arches, back teeth are extracted at an earlier appointment to allow partial healing, which gives Dr. Wilson a more accurate model for positioning the front section of the arch.

Why Immediate Dentures Become Looser Over Time
Bone Grafts at Extraction to Reduce Ridge Shrinkage
It’s possible to slow down how much the ridge shrinks after extraction. Placing bone graft material into extraction sockets at the time of removal occupies space and stimulates new bone formation, reducing the amount of ridge lost during healing.
Socket preservation is most effective when graft material is placed at the time of extraction, while the socket walls are still intact. This is a decision to make before extraction day, not after.
Improving Denture Retention with Dental Implants
A complete denture can also be anchored to dental implants to significantly improve retention and stability. An implant is a titanium post inserted into the jawbone that bonds with the bone through osseointegration, functioning as an artificial tooth root. Once integrated, the denture connects via a snap or screw attachment, preventing it from loosening during chewing or speaking. Standard implants and mini-implants are both options; mini-implants are sometimes used for lower dentures where available bone volume is limited. Implant-supported retention directly addresses the most common long-term complaint among denture wearers: looseness.
What Makes a Denture Successful: Appearance, Function & Fit
Three clinical requirements determine whether a denture succeeds: natural appearance, function, and retention and stability. With immediate dentures, all three are harder to predict because a wax try-in can’t be performed. In a conventional denture process, teeth are set in wax and placed in your mouth so you and the dentist can evaluate and adjust their position before the final prosthetic is processed.
With an immediate denture, your natural teeth are still present when impressions are taken, so this step is skipped. Adjustments after placement are a normal part of the process.
What to Expect in the First 24 to 48 Hours
The denture should generally remain in place for the first 24 hours after placement. Removing and reinserting it during peak swelling can be difficult and painful. Swelling from the extractions may cause the denture to feel tight initially; as swelling resolves over the first few days, the fit will feel progressively less secure. Follow-up adjustment appointments are a standard part of immediate denture care, not a sign that something went wrong.
